State Fundraising Notices

Although our financial report is always sent free to anyone requesting a copy, certain states require us to advise you that a copy of our financial report is available from them.

The Muscular Dystrophy Association ("MDA") is the #1 voluntary health organization in the United States for people living with muscular dystrophy, ALS, and related neuromuscular diseases. For over 70 years, MDA has led the way in accelerating research, advancing care, and advocating for the support of our families. MDA's mission is to empower the people we serve to live longer, more independent lives.

If you are a resident of the following states, you may obtain information directly by contacting:

Florida: A copy of the official registration and financial information may be obtained from the Division of Consumer Services by calling toll-free within the state. Registration does not imply endorsement, approval, or recommendation by the state. MDA’s registration number in Florida is CH308.

Georgia: The following information will be sent upon request: (A) A full and fair description of the programs and activities of MDA; and (B) A financial statement or summary which shall be consistent with the financial statement required to be filed with the Secretary of State pursuant to Code Section 43-17-5.

Maryland: Copies of documents and information submitted by MDA under the Maryland Solicitations Act are available for the cost of copies and postage from Maryland's Secretary of State office by calling 1-410-974-5534.

Mississippi: The official registration and financial information of MDA may be obtained from the Mississippi Secretary of State’s office by calling 1-888-236-6167.

New Jersey: Information filed with the Attorney General concerning this charitable solicitation and the percentage of contributions received by the charity during the last reporting period that were dedicated to the charitable purpose may be obtained by calling 1-973-504-6215. New York: New York residents may obtain a copy of MDA’s annual report by writing to the Office of the Attorney General, Department of Law, Charities Bureau, 120 Broadway, New York, NY 10271.

North Carolina: Financial information about MDA and a copy of its licenses are available from the State Solicitation Licensing Branch by calling 1-888-830-4989.

Pennsylvania: The official registration and financial information of MDA may be obtained from the Pennsylvania Department of State by calling toll-free 1-800-732-0999.

Virginia: A financial statement for the most recent fiscal year is available upon request from the State Division of Consumer Affairs by calling 1-804-786-1343.

Washington: You may obtain additional financial disclosure information by contacting the Secretary of State by calling 1-800-332-GIVE.

West Virginia: West Virginia residents may obtain a summary of the registration and financial documents from the Secretary of State, State Capitol, Charleston, WV 25305.
